SOLICITATION REQUEST FOR QUALIFICATIONS

The Aspen Springs Metropolitan District (ASMD) hereby solicits responses to this Request for Qualifications (RFQ) from General Contractors interested in bidding on the Aspen Springs Fill Station Project, more particularly described below, located near Pagosa Springs, Archuleta County, Colorado. The ASMD will request bids from only those General Contractors qualified to perform the work, based on responses to this RFQ. The ASMD intends to issue Bid Documents on or about April 29, 2013, with a bid opening on or about May 13, 2013. Construction will commence on or about May 15, 2013.

Responses to this RFQ, at a minimum, shall include:

1. A company profile and history relevant to projects of this scope and size, including organizational structure, evidence of bonding capability, and financial stability.

2. Evidence of successful completion of at least two projects of this scope and size within the past five years, with verifiable references.

3. Resumes of key staff members, their assignments, educational background, and experience as it relates to projects of this scope and size.

4. Statements of qualifications for proposed sub-contractors, if any, involved in more than five percent of the project scope.

5. Evidence of ability to staff the project, including sub-contractors, in order that the project may be completed in a timely manner.

The purpose of the project is to construct a Water Fill Station over an existing well head. Some of the components of the project will encompass excavation, reinforced concrete, electrical, plumbing, pump and pay station installation, instrumentation and controls, wood framing, roofing, waterproofing, NSF61 coatings application and metal door installation.

2. You are entitled to have an attorney in any and all juvenile proceedings, or you may waive your right to an attorney. If you desire to be represented by an attorney, you should proceed immediately to obtain one and ask the attorney to notify the Court of his/her name and address. If the Court finds you cannot afford to pay an attorney, and you wish one to represent you, you are entitled to a Court-appointed attorney.

3. You are entitled to a jury trial.

4. Upon the filing of a Petition, which may affect you, the court is required to give written notice of the date of hearing by Summons, unless you have signed a waiver of notice, in which case you may be notified by letter.

5. You have the right to present your own witnesses and to question any witness who appears at the hearing.

6. At the end of the hearing when the Court has made a decision, you have a right to appeal the Court’s decision to the appropriate Colorado Appellate Court.

7. If the Petition is sustained, the Court may make orders concerning the care, custody, control and discipline of the minor children, including commitment of said children to an institution.

Pursuant to C.R.S. § 19-3-502(3), termination of the parent-child legal relationship is a possible remedy available if this petition alleging that a child is dependent or neglected is sustained. A separate hearing must be held before such termination is ordered. Termination of the parent-child legal relationship means that a child who is the subject of this petition would be eligible for adoption.
